Referral admin never shows up as a line on your P&L, so it feels free. It isn't. Between front-desk time, lost referrals and risk, it's quietly one of the most expensive things in the practice — you just never get an invoice for it. Here's the rough maths.
As a practice owner I'd never actually counted this until I tried to fix it. The numbers surprised me, so it's worth doing for your own practice.
1. The time
Every referral that goes out by email generates admin: typing it up, attaching imaging, chasing receipt, fielding "did you get it / did they book" calls. Say that's 15–20 minutes of front-desk time per referral once you count the back-and-forth. Across even a handful of referrals a week, that's hours a month of paid time spent confirming things a system should confirm automatically.
2. The lost referrals
This is the big one, and it's almost invisible. A referral that's never received, never booked, or quietly dropped is a lost patient. If you're a practice that receives referrals — implants, ortho, oral surgery, CBCT — each lost referral is income that simply never arrives. One missed implant referral a month is a serious number over a year.
3. The risk cost
No audit trail of where patient data and imaging went is a standing risk. Most of the time it costs nothing — until the one time it costs a complaint, a delay, or an awkward conversation with the GDC or ICO. You're effectively self-insuring against it with nothing but hope.
4. It compounds
The hidden bit: poor referral experiences quietly erode the relationships your practice runs on. GDPs stop referring to the specialist who never confirms receipt. Patients who fall through the gap don't come back. None of it shows up as a cost — it shows up as growth that never happened.
